Open enrollment runs from November 15th to December 31. If you
are new to Medicare or have recently lost your coverage, you may
not have to wait for open enrollment. Please call 1-800-794-7322
for more information. Below you'll find links for the Freedom
Blue PPO paper application, online application, as well as
complete Freedom Blue information.
